Release for your upcoming Chad Wonder Magic Show

Name Badges: You’ll see in the releases I ask you to kindlty have you child(ren) wear a sticker with their name and a geometric shape. The shape helps me recognize your release level at a later date when developing media and doesn’t single out any of the kids.

Media Release: Since many shows (and all virtual shows) are recorded and/or pictures are taken I occasionally use them in marketing, I never identify children and unless a child is an on stage assistant, their appearance in media is generally brief and rarely the focus. There are three different releases so please read each one carefujlly. It’s great if you choose a stndard/full release since it’s easier for me 😉

COVID-19 Release. The show is set up to respect social distancing between myself and the kids as possible. Please speak up if you feel anyone is unsafe. I always wear a mask with kids on stage but I do NOT when I’m on stage alone. During two tricks, I hand each kid a prop that was touched by myself and then by each child. They are sanitized before the show but not between touchs. You may have your kiddo opted out of this activity in the COVID-19 section on the form.

In consideration for no value received, receipt whereof is acknowledged; I hereby give Richard C Darnell (aka Chad Wonder) and Chad Wonder Magic, Inc including marketing partners and any possible future iterations of Chad Wonder Magic, Inc, the irrevocable right and permission to publish and use, in perpetuity and throughout the world in any media known or not yet known, pictures or video of me and/or my minor.  Your image(s) may be used in whole or in part, composite or retouched in character or form, in conjunction with and identified only by a fictitious name for any purpose as it relates to Chad Wonder Magic, Inc or his business entities.

If the person photographed is under 18, I certify that I am his or her parent or legal guardian and I give my consent without reservation to the foregoing on his or her behalf.

I have chosen my preference for Covid-19 above. Regardless of a Yes or No, I hereby release Richard C Darnell (aka Chad Wonder) and Chad Wonder Magic, Inc including any and all partners, prop manufacturers and suppliers from all liability resulting from a Covid-19 incident with my child, myself or any guest I bring to the event.

Your image(s) may be used in whole or in part as defined by your limiting choice below, composite or retouched in character or form, in conjunction with and identified only by a fictitious name and may not be featured exclusively in the media. for any purpose as it relates to Chad Wonder Magic, Inc or his business entities
